In September, 2020, Nanning was listed as one of the 14 national pilot cities to expand the long-term care insurance system and the only pilot city in Guangxi Zhuang Autonomous Region. Under the unified deployment and careful guidance of the National Medical Insurance Bureau and the Ministry of Finance to expand the long-term care insurance system, Nanning Medical Insurance Bureau has conscientiously implemented the strategy of strengthening the capital of the autonomous region and the deployment arrangements of the municipal party committee and government. After in-depth and detailed exploration and scientific and systematic demonstration, the long-term care insurance system in Nanning was implemented on March 1 this year. After the system landed smoothly, the policy effects in supporting people’s livelihood, releasing employment vitality and empowering industrial development have become increasingly apparent, and achieved phased results. Below, I will make a brief briefing on the situation.

First, the main practices and results

Promoting the exploration of long-term care insurance system is an important measure taken by the CPC Central Committee and the State Council to deal with a series of social and livelihood problems brought about by the rapid aging of China’s population, and it is also an important institutional arrangement to improve people’s livelihood and well-being. Nanning has promoted the establishment of the long-term care insurance system, with the attribute of social mutual aid security, optimized the integration of resources as the "skeleton" and urged the system construction as the "wing". Since it was put into operation for more than 8 months, it has achieved remarkable results:

The first is to make up for the vacancy in the social security system. For a long time, under the background of the implementation of the family planning policy for many years, the trend of miniaturization of family units, the aging process of the population, and the rapid development of economy and society, the disability of family members has brought heavy burdens to many families, and the problem of insufficient social security has become more and more prominent. As of October 31st, 2021, the long-term care insurance system in Nanning has covered 1,540,600 insured employees (including 1,290,600 in-service employees and 250,000 retired employees), and 3,487 people have been assessed according to the application, of which 2,689 people meet the standard of enjoying treatment. Nanning’s long-term care insurance system provides the insured persons who have been assessed as severely disabled with the necessary nursing services and double guarantees of funds on a monthly basis, which, to a certain extent, reduces the expenditure of disabled persons’ families on purchasing long-term care services, brings standardized and professional long-term care services to disabled persons, effectively reduces the burden of long-term care needs of disabled persons due to old age, illness, disability and other reasons, and focuses on solving the basic care security needs of severely disabled persons.

The second is to establish a financing model with equal rights and responsibilities. First of all, unlike other cities in China, which mainly rely on medical insurance funds and financial subsidies, Nanning focuses on building long-term insurance into a stable and sustainable independent insurance, taking the lead in implementing independent financing in the country, and the premiums are shared by units and individuals in the same proportion, reflecting the principle of equal rights and responsibilities and social assistance. Secondly, break through the information barriers of medical insurance and tax departments, and take the lead in realizing the synchronous collection of the payment part of long-term insurance units and the medical insurance premiums of employees in the country, and the individual payment is realized by transferring personal accounts. Third, in view of the problem of socialized management of retirees and no unit payment, we should take the lead in exempting the unit payment part of retirees and reducing the financial burden. Fourth, in order to further optimize the business environment and ensure that the long-term insurance system is implemented without increasing the burden on enterprises. Nanning took the initiative to adjust the enterprise unit payment rate from 7.3% (employee medical insurance+maternity insurance) to 6.95% (employee medical insurance+maternity insurance+long-term insurance), and the overall rate was 0.35 percentage points lower than that in 2020. Moreover, the insured employees were upgraded from double security to triple security, and the burden of enterprise insurance payment was directly reduced by more than 337 million yuan by October 2021, effectively alleviating the double burden brought by epidemic prevention and economic downturn.

The third is to empower the long-term care service industry. In order to solve the problems of "unable to find, difficult to find" and "nonstandard and irregular" in the nursing service industry, we took the opportunity of implementing the pilot project of long-term nursing insurance system, implemented the access evaluation mechanism for the nursing service institutions in Nanning, and established a diversified nursing service supply system that takes care of both home care and institutional care in form and life care and medical care in content. The long-term nursing service institutions shall be managed by agreement, and the access conditions of excellent chain enterprises in other places shall be appropriately relaxed to accelerate the landing of service resources and accelerate the balance between supply and demand. For nursing service personnel, the medical insurance department should strengthen the linkage with civil affairs and health care, strengthen training and supervision as the starting point, and introduce corresponding management measures to improve the service ability and level of the industry. By October 2021, a total of 110 applications for fixed-point access assessment had been accepted, and 87 nursing service institutions met the access requirements, attracting 24 home-based nursing service chain institutions with mature operation experience in first-tier cities in China to enter our city for the first time, such as Fushoukang (Shanghai) Medical Pension Service Co., Ltd. and Yi (Guangzhou) Healthy Pension Industry Development Co., Ltd., which set up branches in Nanning. While investing in people and property, More importantly, it has also brought advanced management concepts and means, which has played a very good role in promoting the quality and efficiency of nursing service industry in Nanning, stimulating the employment and entrepreneurship market, and promoting the structural reform of the supply side of the pension industry.

The fourth is to establish an agency service system that is jointly built and shared by the government and market players. Under the framework of national guidance, starting from improving management efficiency and handling efficiency, under the premise of ensuring fund safety and effective supervision, first, set up functional institutions. In the municipal medical insurance agencies to increase the long-term insurance management functions, set up a separate nursing insurance management department, responsible for the city’s handling management. Second, the 13 service points of the city’s medical security agencies have opened long-term insurance windows simultaneously, providing convenient services for the insured in the city to be handled by the whole city and accepted by one window. Third, establish a social and business cooperation mechanism. Through government purchasing services, we will introduce three professional commercial insurance institutions and set up a special long-term insurance service center to provide policy consultation, disability assessment, service organization, fee payment and other services for the insured in the city, further improve the accessibility of long-term insurance services, effectively use the integrated resources such as insurance services, comprehensive supervision and back-office support of commercial insurance institutions to participate in the joint construction of people’s livelihood security, and release the direct benefits of enterprise development, government efficiency and mass convenience.

Generally speaking, the long-term care insurance system in Nanning has built a clear institutional framework, a financing mechanism with equal rights and responsibilities, an objective and fair evaluation system, a scientific and reasonable treatment standard, an efficient and convenient information system and a standardized and orderly handling system, which has made up for the vacancy of the social security system in our city. The implementation of this system is not only conducive to reducing the double burden of disabled people and their families’ economy and affairs, but also to resolving the social risks of the aging population demand. At the same time, it broadens the employment channels for aged care services and nursing practitioners, and promotes the coordinated development of aged care services, health industry and insurance industry. Since the pilot implementation of the long-term insurance system, it has run smoothly, achieved remarkable results and received good response from all walks of life. Some people sent letters and calls, some sent banners, thanked the party for its good policies, thanked the government for its care, and praised the long-term care insurance for improving the quality of nursing services and happiness index of disabled people.

Second, the next work plan

The first is to expand and extend the scope and connotation of protection. On the basis of consolidating the effectiveness of the pilot project, taking the establishment of an evaluation index system for long-term care needs in Nanning as a breakthrough point, and considering the disease distribution, economic and social development level, fund financing and treatment in Nanning, the care needs and corresponding care levels of disabled and semi-disabled people are reasonably defined, and the corresponding nursing service methods, assistive devices needs and the amount of guarantee funds related to related services are determined according to the results of disability evaluation. At the end of 2022, the policy of including the rental service fee of assistive devices into the payment scope of long-term care insurance fund is studied and introduced.

The second is to deepen the resource integration and policy convergence of relevant industry sectors. At the present stage, there are multi-faceted and multi-dimensional policy arrangements from the central government to the local government to cope with the aging population and meet the health needs of the masses, and the finance has also invested a lot of money. At present, many commercial insurance institutions have shifted their development focus to the field of healthy old-age care, but the funds and policies are scattered in related fields and it is difficult to form a joint force. Promoting and participating in the construction of long-term care insurance system is not only to improve the social security function, but also an important means for the government to regulate the allocation of resources in related fields, and to promote the supply-side structural reform, safeguard social equity and protect the most basic interests of the people. It requires the joint planning, coordinated development, mutual integration and mutual promotion of various government departments and enterprises.

The third is to improve supporting policy support and innovate the management of handling services. At present, the focus of the pilot work of the long-term care insurance system in our city has changed from ensuring a steady landing to deepening the pilot reform. To deepen the pilot work, it is necessary to promote the coordination of government departments, focus on system vacancies and market demand from the perspective of industry management, and constantly improve the system to provide more comprehensive and better guarantee services for the masses. At the same time, we will deepen the cooperation between medical insurance and commercial insurance in building a long-term care insurance handling service system, adhere to the problem-oriented, demand-oriented and effect-oriented, further clarify the functional orientation of long-term care insurance, improve the demand assessment mechanism, improve the service supply capacity, continue to strengthen the close cooperation between cities and counties, do a good job in the innovation of information-based integrated management means such as "unified management by one network" and "unified management by the whole city", empower the management of long-term care insurance, and push the mode of government-enterprise co-construction in the field of building public service system to a new level.

Below, I will make a major policy interpretation on the recently issued Interim Measures for the Management of Nursing Service Personnel in Nanning Long-term Care Insurance:

First, the connotation of long-term care insurance system pilot

The Fifth Plenary Session of the 18th CPC Central Committee proposed to "explore the establishment of a long-term care insurance system". The Fifth Session of the 19th CPC Central Committee proposed to "steadily establish a long-term care insurance system". In March 2021, the 14th Five-Year Plan for National Economic and Social Development in People’s Republic of China (PRC) and the Outline of Long-term Objectives in 2035 adopted by the National People’s Congress and the National People’s Congress made arrangements for the establishment of a long-term care insurance system. Promoting the exploration of long-term care insurance system is an important measure for the CPC Central Committee and the State Council to deal with a series of social and livelihood problems caused by the rapid aging of China’s population.

For ordinary people, the long-term care insurance system is an important supplement to life-cycle health protection, providing economic compensation and long-term care service guarantee for the insured who cannot take care of themselves due to old age, illness, disability and other reasons.

With the deepening of the pilot work, how to mobilize resources from all sectors of society to provide professional, high-quality, efficient and stable long-term care services for disabled insured persons is the key to the deepening and refinement of the long-term care insurance system pilot.

Second, the background of the introduction of the interim measures for the management of long-term care insurance nursing service personnel

According to the deployment of the national pilot project to expand the long-term care insurance system, after careful and pragmatic research, the long-term care insurance system has been implemented in Nanning since March 1, 2021. Since the pilot operation for nearly 8 months, the city has actively explored the institutional framework, policy support, handling services and standardized management. Up to now, the city has accepted 110 applications for admission assessment of designated institutions, and 87 nursing service institutions meet the admission conditions, covering medical institutions, old-age care institutions and home care service institutions, and registered 1,330 long-term nursing service personnel. The establishment of the long-term care insurance system has reduced the double burden of family economy and affairs brought by the long-term care needs of the insured severely disabled people in our city. The timely introduction of the interim measures for the management of long-term care insurance nursing service personnel is conducive to further standardizing the long-term care service behavior, promoting the formation of long-term care service industry, driving a group of nursing service institutions to enrich the market supply of long-term care services, forming healthy competition and beneficial interaction, and further promoting the process of institutionalization and standardization of long-term care services in our city.

Third, the main content

(1) General requirements. The management objective of the present Measures is to ensure that those who enjoy long-term care insurance in our city can obtain safe, standardized and effective long-term care insurance basic nursing service items and services, consolidate the socialized service support of the long-term care insurance system, avoid developing into a welfare system, and put forward specific requirements for those who provide long-term care services.

(2) Scope of application. These Measures are applicable to those who have been recorded by the medical insurance agencies and long-term insurance contractors in our city, signed relevant management contracts with the designated nursing institutions in our city, and provided Nanning long-term insurance basic nursing service items and services for those who enjoy long-term insurance treatment (collectively referred to as long-term insurance nursing service personnel).

(3) Access scope. Including those who provide in-house nursing and on-site nursing services in designated nursing institutions in our city, who have signed labor, labor and other related subordinate management contracts with their institutions (social caregivers) and severely disabled people or their entrusted agents, who have the ability to provide full-time home (or on-site) long-term nursing services and signed directional nursing labor contracts with long-term nursing institutions (exclusive caregivers). At the same time, under the age of 65 (inclusive), in good health, without any infectious diseases.

The main considerations are as follows: First, rationally distinguish the functional orientation of family care and professional care, further optimize the allocation of long-term care service resources, ensure that the severely disabled people in long-term care insurance get professional and high-quality long-term care protection, and at the same time, provide protection for promoting the socialized use of long-term care service resources. Second, according to Article 15 of the Labor Insurance Regulations and Item 1 of Article 1 of the Interim Measures on Retirement and Resignation of Workers, male workers and employees should retire and enjoy retirement pension subsidies when they reach the age of 60 and female workers and employees reach the age of 50. Accordingly, male farmers who have reached the age of 60 and female farmers who have reached the age of 50 can be regarded as "incapable of working". Combined with the latest classification standards for the elderly, the age of long-term care service personnel should be relaxed to 65; At the same time, the physical health of severely disabled people is extremely poor, and their resistance is weak. Therefore, those who provide long-term care services must ensure "good health and no infectious diseases".

(4) personnel management. The specific requirements for the management of long-term nursing service personnel relying on designated nursing institutions were clarified, and the management responsibilities of medical insurance agencies, long-term nursing contractors and designated nursing institutions for long-term nursing service personnel were clarified. The management object is required not to leave the post without approval.

(5) Personnel training and assessment. Long-term nursing service personnel should regularly participate in professional ethics and business training organized by designated nursing institutions, and the annual training duration should be no less than 20 class hours (one class hour per hour). The main considerations are: quantifying the training duration will help to ensure that the nursing skills of service personnel are optimized and easy to assess.

(6) Others. If the nursing service personnel who have been included in the management in the past are inconsistent with these measures, a three-month transition period will be set up and gradually standardized. Mainly consider promoting the stable and smooth transition of this approach.

The long-term care insurance system in Nanning adheres to the basic principle and development concept of social security and mutual assistance, and makes in-depth exploration on the path of coordinating all social forces to solve the contradiction of long-term care demand. The goal is to establish and improve the long-term care insurance system into a social security system with clear positioning and strong compensation, and provide dual support for the families of disabled people. It needs the active participation, strong support and tolerance of the whole society.

First, fine and efficient, high standards to promote long-term care insurance handling services

(a) standardized and orderly, and comprehensively improve the level of handling services. Long-term insurance relies on the medical insurance agencies at all levels in our city to implement the handling management. As the long-term insurance contractor, commercial insurance institutions, under the guidance of the medical insurance department, carry out specific handling services within the responsible area according to regulations. Always adhere to the principle of combining government leadership with market mechanism, and effectively promote the reform and innovation of medical security governance model. The first is to formulate unified handling procedures. The Municipal Medical Insurance Center organized the formulation and implementation of the unified long-term insurance handling service process in the city, actively explored and formulated the long-term insurance handling procedures and service standards at the initial stage of the system pilot, defined the operational processes and requirements of acceptance, handling and auditing, and made detailed and comprehensive arrangements for key handling services such as disability assessment, nursing service and cost settlement. The second is to strengthen the guidance and supervision of the contractors. In order to ensure that the daily handling work is carried out in an orderly manner, the Municipal Medical Insurance Center comprehensively controls the progress of various handling work, sorts out the difficult points in time, strengthens the guidance and supervision of the long-term insurance contractors, and aims at efficient and convenient service, constantly optimizes the handling service process in practice, and comprehensively improves the handling service satisfaction. The third is to strictly control the assessment of disability. The Municipal Medical Insurance Center selected 238 medical experts with high professional quality and familiar with the long-term care insurance policy from designated medical institutions to be included in the long-term care insurance assessor database for unified training and assessment, which provided a strong guarantee for the implementation of disability assessment. Continuously optimize the assessment workflow and promote the establishment of standardized work flow for disability assessment.At the beginning of September, the new evaluation standard was quickly launched in the whole city, and the evaluation work was carried out in strict accordance with the "Evaluation Standard for Long-term Care Disability Level (Trial)" jointly issued by the Office of the National Medical Insurance Bureau and the General Office of the Ministry of Civil Affairs on July 16 (No.37 [2021] of the Medical Insurance Office). The old and new standards were used iteratively and orderly, and the evaluation work was fair and objective.

(2) Take multiple measures to ensure that service supervision is in place. The municipal medical insurance center organizes the handling force to actively carry out the policy and business training for designated nursing service institutions, continuously strengthen the supervision of designated nursing service institutions, and urge designated nursing institutions to improve their management and service level, improve the quality of nursing service, so that severely disabled insured personnel can get a better nursing service experience. First, strengthen daily training and guidance. There were 10 trainings on long-term insurance policy consultation, organization management and nursing service, and information system operation for designated nursing service institutions, involving about 500 people. Guide the designated nursing service institutions to accurately grasp the connotation of the long-term insurance system, find the correct positioning, adhere to the people’s health as the center, solve the basic nursing security needs of severely disabled people as the basis, pursue the provision of safe, reasonable and high-quality long-term insurance nursing services, focus on the supply of high-quality nursing services, carefully organize, set an example, standardize operations and compete fairly. The second is to implement the supervision of designated nursing service institutions. Strengthen the agreement management of designated nursing service institutions through the way that the contractors independently carry out daily audit inspections, and the medical insurance departments jointly carry out special inspections with relevant departments such as civil affairs and health care. On-the-spot inspection of archives and return visits to nursing objects, focusing on the supply of nursing services, scientifically setting inspection contents and indicators, recording inspection problems on the benchmark table and urging rectification to fully protect the legitimate rights and interests of the treated objects. The third is to strengthen the management of nursing service personnel.City medical insurance center is responsible for the establishment of a unified information base of nursing service personnel in the city, relying on the integrated information platform to implement real-name registration system filing management for all nursing service personnel, and comprehensively record the basic information and training and assessment of personnel. At the same time, the designated nursing service institutions are responsible for establishing the registration management system and evaluation system of nursing service personnel in the institutions, and implementing professional and standardized daily business training and professional ethics training for nursing service personnel in the institutions to meet the multi-level and diversified nursing service needs of severely disabled insured persons.

(3) Strengthen publicity and actively guide public expectations. Formulate a publicity work plan, actively plan various publicity work, innovate publicity methods, comprehensively use various platforms, outlets and media resources to carry out publicity activities, actively and positively publicize the long-term insurance policy, properly respond to public concerns, deepen the understanding and support of all sectors of society for the pilot work of the long-term insurance system, broadly gather social consensus, and lead the pilot work of the system to a deeper level. First, online publicity is uninterrupted.
